Incidents, hazards and near misses in one place

HSE records incidents, near misses, risk assessments and inspections. It calculates safety KPIs from these records and shows the source of working hours.

A near miss is reported for Aisle 3.

What it does

  1. 01

    Near misses and incidents

A reported near miss creates a review task in Tasks. Incidents carry corrective actions, comments and a change log.

  2. 02

    Hazards and controls

The hazard card calculates risk as probability times severity. Finishing a control closes its task.

  3. 03

    Safety KPIs from records

LTIF shows its formula and the source of working hours. With no hours, it says what is missing.

Where the data comes from

  • The safety team enters incidents, near misses, assessments, inspections, hazards and PPE issues.
  • Safety andons from MES and high-severity floor alerts can open an incident automatically.
  • Working hours for LTIF come from People attendance.

Works with

Good to know

  • HSE keeps no safety training register. Courses are in LMS and are not linked to this screen.
  • Work permits are issued in People. HSE shows that screen and keeps an archive of old permits.
  • Incident forms have no OSHA or RIDDOR classification. Near-miss and PPE records can be closed but never deleted.
